Leadership North Texas may have more applicants than can be accepted. To assist the Selection Committee in giving all applicants the fairest consideration, complete this application as accurately and as completely as possible.

Please follow these guidelines:

• Application should be neatly typed or printed. (May be downloaded from www.leadershipnorthtexas.org)
• Don't be modest -- list all activities, offices, etc., where requested.
• A resume may be attached to enhance the application, but not to serve in lieu of.
• Three recommendations are required.
• Please make two additional copies of the recommendation form and forward one form to each of your three endorsers. Please do not send more than three.
• Include a $25 non-refundable application fee (either check or credit card).
• Forward one Recommendation Form to each of your three endorsers. Each endorser should mail the completed form before May 21, 2010 to:

• Enclose a recent photograph with your application. Do not affix photo to the application form.
• Please choose a (Black and white, 3 X 5 or smaller) photo suitable for use in the LNT Class Directory. (A clearly identified digital photo file may be sent by email to: Kimberly@ntc-dfw.org.)

• Attach Application Fee of $25, check or money order payable to the North Texas Future Fund.

Your application, $25 non-refundable application fee, photo, signed calendar of times and dates, and three recommendation forms must be received by Leadership North Texas no later than May 21, 2010.

Incomplete or late applications will not be considered. NO exceptions. Do not enclose tuition payment with your application. The 2009-2010 LEADERSHIP NORTH TEXAS Class will be announced in mid June.
